Rates (Rental fee and deposit must be paid seven (7) days prior to the event):

Rental Fee:  $150.00 per day                                                                  Refundable Security Deposit:  $125.00 (Refer to policies)

Policies

1. The complete Rental Agreement, Deposits, and Rental Fee are due 7 days prior to (in advance of the rental date and are deposit upon receipt into SMVNA bank account. There are no exceptions to the policy.
2. Refundable deposits will be returned within 10 days if all rules, cleaning and rental guidelines have been met and no damage has occurred. There are exceptions to the policy.
3. In the event of a “no-show” renter, the SMVNA will retain the Rental Fee. However, the SMVNA will refund the Security Deposit within 10 days.
4. Immediately before the rental event, a SMVNA representative and the person responsible for rental will review the rental guidelines and complete a brief inspection of the premises. Only an authorized member of the St. Martin Village Neighborhood Association (SMVNA) will grant access the Charles Quillings Center.
5. The SMVNA reserves the right to enter the building during a rental event.
6. The St. Martin Village Neighborhood Association does not discriminate on basis of gender, or gender identity, race or color, ethnic or national origin, political affiliation or belief, religion or disability.
7. The individual renter must be 25 years of age or older to rent the Charles Quillings Center.
8. List of names, address, and telephone of each person in attendance.

Cleaning Policies Agreement  

The renter of the Charles Quillings Center is responsible for the following:

1. Trash – All trash must be removed for the center and placed in the dumpster.
2. Floors must be swept and any spills cleaned. Floor must be left as found upon arrival.
3. Restrooms – Restrooms must be left in the condition they were found during initial inspection.
4. Please report any maintenance or plumbing problems to a SMVNA representative.
5. Tables and chairs must be unbroken, accounted for returned to their original placement. If any table or chair is damaged the amount to replace the item will be deducted from your deposit. It is your responsible to clean the tables and chairs.
6. Counter tops, sinks, stove top and oven, refrigerator, microwave, tables and chairs must be wiped clean and in the same condition as found upon arrival.
7. Food – Remove all food, beverages, ice, etc. from the Charles Quillings Center prior to your departure.
8. Decorations –NO HANGING ANYTHING Failure to remove all/any tape, streamers, gum or other items found on the walls or floors, poles, or ceilings result in the loss of the security deposit.
9. Grounds – The ground outside the shelter will be free of liter.

If renter fails to meet the standards detailed above or on Page 1, the security deposit will not be returned.

By signing the documents provided, the renter is officially informed that failure to follow the rules listed in the (1) Policies, (2) Charles Quillings Facility Rules, or (3) Cleaning Policies Agreement can result in the immediate cancellation of rental, forfeiture of the Security Deposit and the renter may be denied use of the Charles Quillings Center in the future. The permit holder shall release, absolve, indemnify, save and hold harmless the St. Martin Village Neighborhood Association and Lexington-Fayette Urban County Government, the Division of Parks and Recreation, and their agents and employees, from against all liability, claims, and demands on account of personal injuries ( including, without limitation to the foregoing, workmen’s compensation and death claims) or property loss or damage of any kind whatsoever, to the permit holder or any individual(s) who is injured at the function for which the facility is being used, which arise out of or is in any manner connected with the performance of this contract, regardless of whether such injury, loss, or damage shall be caused by, the negligence of the permit holder or of the Lexington-Fayette Urban County Government, the Division of Parks and Recreation, or by any of their agents or employees, the Georgetown Street Neighborhood Association, or by accident or otherwise.

I agree to comply with all parts of the Rental Agreement including Rates, Policies, Cleaning Policies Agreement, and Charles Quillings Center Facility Rules.

COVID Wavier

The undersigned acknowledges that LFUCG/St. Martins Village Neighborhood Association has created new protocols and put in place preventative measures to reduce the spread of COVID-19; however, LFUCG/St. Martins Village Neighborhood Association cannot guarantee that anyone utilizing a Park facility will not become infected with COVID-19. By signing this agreement, the undersigned voluntarily assumes the risk of exposure to COVID-19 and that such exposure or infection may result in personal injury, illness, permanent disability, and death.

Storage Room

The storage room is not to be used for a party room.  Otherwise, you will not receive your deposit.
